All of our primary care practices follow a Patient Centered Medical Home model. In this model, patients are members of a team that includes their primary care physician, other caregivers and staff, all of whom work with the patient to manage their care. This team approach enables better coordination of care across several clinical specialties. The medical care team includes:

Coordination across the Mass General Brigham network

We are connected to the other hospitals within the Mass General Brigham network through a single electronic medical record. If you are our patient and you receive care at other hospitals or affiliated physicians within the Mass General Brigham network, this information will be reported back to your primary care physician and care team so that they remain aware of your health.
