After scheduling your appointment with your new primary care physician, new patients will be transferred to our central registration office to complete the registration process. Central registration will request your demographic, insurance and employment information, which is kept confidential and in compliance with HIPAA.

Established patients may also be called prior to appointments to verify and update their insurance and/or demographic information.

This service prevents any unnecessary delays due to incomplete information during your appointment. To report any changes in your insurance and demographic information, please call our central registration office at 877-892-7707. We are open Monday - Thursday 7:30 - 6 p.m., and Fridays 7:30 - 5 p.m.
