Electronic medical records
State-of-the-art technology facilitates patient-centered care. All North Shore Physicians Group physicians make use of the electronic medical record system that connects physicians across the Partners Healthcare network. Electronic medical records enable NSPG physicians to closely track each patient's care and determine the best course of treatment. In many cases, the team proactively reaches out to patients instead of waiting for them to come to the doctor.
NSPG clinical staff are able to effectively manage a patient's chronic conditions through remote monitoring technology. Below is a Better Living piece by WCVB Television reporter Liz Walker, featuring an NSPG practice and NSPG patient.
Remote monitoring programs are offered in a number of primary care practices. Patients become collaborative partners in their own healthcare through programs that track their health and securely share their health data with their provider.
Blood pressure, blood glucose and weight are just a few examples of the type of conditions and data which can be viewed within the patient's medical record. Clinicians gain a more complete picture of the patient's condition and are able to effectively assess his or her status and provide just-in-time care.
